It is the second day of your emergency rotation and you are asked to see Mr. Domquee, a 30 year old male who got into a bar fight while "minding his own business". As you walk into the room, you notice a visibly swollen eye and some large bruises on his arm. While doing your history, he complains that his arm is in a lot of pain and that his vision has recently become blurry. Upon examination he has some proptosis of the eye, decreased visual acuity and increased intraocular pressure. When checking his pupils, you notice they are not responding normally when doing a swinging light test. What are you most concerned about?
